Terms and Conditions -

• Must have used the Coupon Code from the  "most fun you've had all week" Advertisement to purchase your Ticket

• Must Have turned up and  played for at least %50 of the Ticket Time paid for

• Information on this Form must be filled out correctly

• This form must be filled out for each Participant wanting to Claim a refund 

• A Valid activity you did and why you had more fun at that Activity

• Must Lodge Form within 3 Days of Dodgebow Event you participated in
